About the Role:
We are seeking a dedicated Advanced Practitioner to join our integrated Care Team, working alongside Registered Nurses and Social Workers. You will play a crucial role in providing comprehensive care to individuals with chronic kidney disease (CKD) and end-stage renal disease (ESRD), often managing multiple complex health conditions. This position offers the opportunity to improve patients' quality of life in the comfort of their homes and slow the progression of their kidney disease, leading to positive health outcomes. You will be responsible for providing comprehensive care, maintaining accurate records, scheduling appointments, and ensuring timely follow-ups. What You'll Do:
Conduct comprehensive assessments, including annual wellness exams, both in patients' homes and virtually.
Counsel and educate patients and families about available benefits and programs to support their health journey.
Thoroughly document patient information, including chief complaints, diagnoses, medical history, medications, allergies, assessments, and plans.
Coordinate care with primary care providers, specialists, and other necessary services.
Complete all documentation and paperwork promptly and accurately.
Maintain the highest standards of patient care.
Deliver evidence-based, timely care that reduces avoidable hospitalizations, maximizes quality of life, and prioritizes patient health and satisfaction.
Prescribe medications, order tests, and collaborate with patients' physicians.
Drive improvements in patient quality outcomes.
Educate patients about preventive monitoring and collaborate with physician practices to schedule diagnostic testing.
Assist patients in accessing educational resources.
Actively participate in integrated care team meetings.
Assess patient health status, develop and implement treatment plans, and closely monitor patient progress.
Perform other duties as assigned.
What You'll Bring:
Bilingual (English/Spanish) skills are highly preferred.
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
Board certification for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work independently and autonomously.
Access to reliable transportation, a valid driver's license, and car insurance.
Proficiency with medical instruments and equipment.
Strong knowledge of computer-based data management programs and information systems.
Excellent communication skills, both verbal and written.
Solid understanding of all federal and state regulations, including HIPAA and OSHA.
Two or more years of direct patient care experience.
Experience with Managed Care/IPA/Health Plans.
Experience conducting annual wellness visits (virtually or in-home).
